Healthcare & systems / systemsincontext™

Your practice.
Backed by our healthcare scheme navigators.

In Context gives your practice access to healthcare professionals who help patients navigate complex services and support options. Practical support for your patients, with relevant updates for your practice.

For GP, allied health and specialist practices in Sydney and South West Sydney. A defined monthly partnership for the work between health, disability, aged care and other support systems.

The gap between services

Several providers.
Who holds the whole picture?

A patient can have the right people around them and still be unsure what happens next.

We provide professional support when navigating services becomes difficult. Your practice and patients have someone to contact for help.

Support is tailored to each person’s circumstances and the services available to them.

  • Medicare and mental health pathways
  • NDIS
  • Aged care
  • Private health
  • Victims Services
  • Insurance, superannuation and TPD
  • Workers compensation
  • Veterans’ supports
  • Equipment schemes
  • Carer and community supports
  • Centrelink and other government supports

Pathways depend on the person’s circumstances, eligibility and available services. Scheme owners retain their funding and approval decisions.

Two services. A clearly agreed scope.

Guidance when it is complex.
Support when it is needed.

Cross System Case Management™

Support through complexity.

Additional support for complex situations where ongoing assistance is needed.

The support, duration and fees are agreed before work begins.

Each team keeps its role. Clinicians provide treatment. Practice managers run the practice. Reception handles routine patient arrangements. Support coordinators, recovery coaches and scheme case managers continue their existing roles. We take on the agreed work across them.

Working alongside NDIS support coordination

NDIS support coordination remains with the person’s support coordinator. Systems in Context works alongside support coordinators, psychosocial recovery coaches and existing providers when healthcare and other support pathways overlap.

Our Cross System Care Navigation™ and Cross System Case Management™ services provide additional support across systems, with an agreed scope and the person’s consent.

Workers compensation and other healthcare pathways

When workers compensation, Medicare, NDIS or private health pathways overlap, practices can discuss the person’s navigation needs with us. We confirm the relevant service and scope before accepting a referral.

The insurer, employer, treating clinicians and any appointed workplace rehabilitation provider retain their roles. Funding and approvals remain with the responsible scheme.

Who we work alongside

GP and allied health practices, specialist teams, hospitals, support coordinators, psychosocial recovery coaches, case managers, aged care providers and other relevant services.

Health services, government and organisations can also discuss a scoped partnership. We agree responsibilities, consent, capacity and communication before starting.

Navigation and treatment have different roles

Systems in Context provides navigation and case oversight. It does not deliver treatment or decide scheme eligibility. Clinical care remains with the person’s chosen providers.

Health in Context is our clinical services division. Using Systems in Context does not require using Health in Context. Provider choice and any relevant interests are discussed openly.

In practice / A fictional example

Meet Kevin.

A person. Several systems.
A story to explore, one click at a time.

Hi, I’m Kevin.

01 / Meet Kevin

Kevin already has a team.

Kevin lives with a physical disability and ongoing mental health challenges. He has NDIS supports and an established healthcare team.

I need some more support.

02 / A new need

Something has changed.

Following a distressing incident, Kevin needs additional psychological support. His clinicians discuss several options with him.

Where do I start?

03 / What next?

Several options can feel overwhelming.

Kevin finds it difficult to understand the different services and funding options. He would like help knowing where to turn.

My practice can connect me.

04 / Partner Practice

His GP practice is an In Context Partner.

With Kevin’s consent, the practice connects him with In Context for professional navigation support.

I have someone to ask.

05 / Support

Help with a complicated situation.

Kevin has someone to contact about his navigation needs. His existing providers continue their roles and his treating clinicians continue managing his care.

The next step feels less overwhelming.

06 / Looking ahead

More clarity for Kevin.

The aim is to help Kevin feel supported in accessing appropriate services, with his consent guiding relevant communication with his healthcare team.

Discuss a practice partnership

Fictional example illustrating a gap between services.

A little context

Kevin’s treating professionals provide care. Schemes and insurers make their own funding and cover decisions. Systems in Context provides navigation and case oversight, with consent and an agreed service scope. Every person’s circumstances are different.

What does the patient pay?

Patients are not charged an hourly navigation fee by In Context for work included in their Partner Practice agreement. Treatment fees and other services remain separate. Access and capacity are confirmed before a referral is accepted.

Patients and families can ask their GP or treating practice whether they are an In Context Partner Practice.

Commercial clarity and clinical independence

No fee splitting, no per referral charges and no interference with clinical decisions. The retainer covers a defined navigation service. Medicare claims, clinical documentation and scheme approvals remain with the responsible professionals and organisations.

A quick practice check

Is system friction
costing your clinic?

Five questions about the work between schemes.

This check identifies pressure points. It does not estimate lost revenue or assess your Medicare billing.

01Do your GPs spend more than 15 minutes a week trying to find external coordinators or advocates?
02Does your reception desk frequently field calls from NDIS planners or Aged Care assessors?
03Do practice managers chase paperwork or updates across several external agencies?
04Does work between schemes delay clinically appropriate care plans, referrals or reviews?
05When a patient has several providers, does your team end up tracking the overall next steps?
