What support would help?

Choose one or more services

We will confirm practitioner availability, suitability and any waiting period before arranging care.

A short description is enough, such as daily routines, getting out in the community or an assessment for a plan review. Please leave detailed medical information out.

For example, a plan review date. We will confirm whether we can meet it.

Contact and funding

Give the details of the person we should contact first. They need to know about the referral and agree to being contacted.

For example, participant, parent or support coordinator.

Provide the email address or phone number needed for the contact method you choose.

For example, an interpreter, written communication, a support person or suitable times.

We will check the funding arrangement and service eligibility with you. A referral does not confirm that NDIS funding can be used.

How we use this referral

In Context Pty Ltd collects these details to review the referral, check service suitability and contact the person nominated below. Authorised In Context staff can access the referral through a private inbox. We record your consent and the date of submission.

Details are stored by our website hosting service, which may process information outside Australia. This form is for first contact and is not a clinical record. Please leave out NDIS numbers, full plans, reports and detailed medical information. We will arrange a suitable way to collect further information if needed.

You can leave optional questions blank. Without the required contact details and consent, we cannot process an online referral. You can call 1300 952 474 to discuss another way to refer.

To request access or corrections, discuss deletion, withdraw consent for future contact or make a privacy complaint, email info@incontext.au. We may need to retain information where required by law. Read our privacy information. Referrals do not subscribe anyone to marketing.

Being a family member, nominee or support coordinator does not automatically give someone authority to consent to health care. If you are unsure, contact us before submitting.
